As the Ego accelerates, a pedestrian begins moving and enters the crosswalk from the other side of the intersection. This activates the rule-based crosswalk logic. Ego determines that it is able to stop before the crosswalk without exceeding our max deceleration threshold, which triggers an in-policy stopping reaction for the crosswalk. The Ego brakes to yield for the pedestrian. The vehicle trailing the Ego accelerates, making contact with the rear-end of the Ego. In the 2.5 seconds between the start of the Ego braking and the collision, the rear vehicle accelerated from 0.2 to 6.1 m/s (0.4 to 13.6 mph) without slowing until after the collision occurred. In response to this incident, immediate mitigations were made, in that the vehicle operators (AVOs) operate all intersections in manual mode. This remained in place until January 12, when we received a solution which instituted a reduction of the 3.5 m/s2 parameter, in order to prevent the Ego from yielding to pedestrians in crosswalks that are still far from the Ego’s lane if such yielding would induce braking.","occurredAt":"2025-12-01T12:00:00.000Z","reviewStatus":"reviewed","modelId":"e107a295-765b-49c3-a41d-40eee29f9d50","deploymentId":"585a9404-9de4-4a55-a533-c53ea3c9559f","companyId":"8f52c3ed-fb8c-4f25-8c75-c7c9f912776d","dataCenterId":null,"aliases":[],"collisionRisk":"none","reviewNote":null,"severity":"minor","rootCauseCategory":null,"rootCauseDetail":null,"affectedUnitsCount":null,"affectedUnitsBasis":null,"remediationStatus":null,"regulatoryAction":null,"regulatoryBody":null,"propertyDamageEstimateUsd":null,"damageEstimateBasis":null,"bodilyInjurySeverity":"unknown","fatalityCount":null,"lossCostClass":null,"sourceQualityTier":null,"oshaRecordable":null,"oshaCaseNumber":null,"rootCauseDisclosed":null,"publicDisclosureDate":null,"insuranceClaimPublic":null,"formFactor":null,"latitude":null,"longitude":null,"autonomyClass":null,"legalOutcomeClass":"unknown","createdAt":"2026-07-12T05:01:44.266Z","updatedAt":"2026-08-23T05:02:31.850Z","model":{"id":"e107a295-765b-49c3-a41d-40eee29f9d50","companyId":"8f52c3ed-fb8c-4f25-8c75-c7c9f912776d","modelName":"May Mobility Autonomous Sienna","slug":"may-mobility-sienna","description":"May Mobility's autonomous, ADA-accessible Toyota Sienna minivan fleet running its Multi-Policy Decision Making (MPDM) autonomy stack for microtransit / robotaxi service.","formFactor":"av","industryVertical":"passenger_transport","environment":"road","maturityStage":"commercial","lifecycleState":"active","supersededByModelId":null,"specs":[{"label":"Platform","value":"Toyota Sienna minivan (Autono-MaaS; first ADA-compliant wheelchair-accessible AV variant)"},{"label":"Autonomy","value":"SAE Level 4 (driver-out commercial service); MPDM re-evaluates scenarios every ~200ms"},{"label":"Scale","value":"~500k rides across ~19 deployments"},{"label":"First driver-out","value":"Peachtree Corners GA (Feb 2025); integrated into the Lyft app in Atlanta"}],"manufacturerSerial":null,"reviewStatus":"reviewed","sources":[{"url":"https://maymobility.com/posts/may-mobility-now-offers-autonomous-driverless-rides-in-peachtree-corners/","title":"May Mobility driver-out service","sourceName":"May Mobility (official)"},{"url":"https://maymobility.com/news/may-mobility-expands-fleet-with-toyota-sienna-autonomos-shuttles","title":"May Mobility expands fleet with Toyota Sienna autonomous shuttles","sourceName":"May Mobility"}],"aliases":["May Mobility Sienna","Toyota Sienna Autono-MaaS"],"collisionRisk":"low","reviewNote":"Reviewed 2026-07-14: source-gated bulk flip (non-empty sources[]).
