NHTSA opens Preliminary Evaluation PE25012 into 2.88 million Tesla FSD vehicles over red-light running and wrong-way driving
On October 7, 2025, NHTSA opened Preliminary Evaluation PE25012 covering an estimated 2,882,566 Tesla vehicles equipped with Full Self-Driving. The probe examines traffic safety law violations including proceeding through red signals and driving against the proper direction of travel while FSD is engaged. At opening, NHTSA counted 58 incidents including 14 crashes and 23 injuries with no fatalities.
Some vehicles ran red lights after coming to a complete stop first, indicating a decision error rather than perception failure. By December 2025 the tally grew to approximately 80 reports.
