On Apr 6 2022 a TuSimple autonomous truck on I-10 near Tucson abruptly swerved left into the center concrete barrier. A test driver re-engaged autonomy before the system was primed, and the truck executed a stale ~2.5-minute-old steering command that should have been flushed; HMI design + command-stale handling were both implicated.
