On April 29, 2025, a five-person crew at the Port Arthur LNG facility in Texas was performing tank formwork jumping at elevation when the climbing formwork system gave way due to an improperly secured bracket connection. Three workers — Reginald Magee, Felipe Mendez, and Felix Lopez Sr. — lost their lives, and two others were injured. Bechtel conducted an internal investigation and cooperated with OSHA, identifying multiple contributing factors including safety culture breakdowns and oversight failures.
