OSHA: Worker fatally crushed by spot-welding robot during maintenance at Unipres Southeast (2022)
At about 9:15 p.m. on September 1, 2022, a 27-year-old precision assembler at Unipres Southeast U.S.A., Inc. was placing welding tips into a welding-tip cartridge for a spot-welding robot when the robot activated and fatally crushed him. The equipment had not been properly secured with lockout procedures. OSHA accident investigation, inspection 1619852.015.
