On June 19, 2026 at approximately 8:03 p.m., a 2025 Tesla Model 3 with Full Self-Driving (Supervised) active crashed into a residence in the 21300 block of Rose Hollow Lane in Katy, Texas, killing 76-year-old Martha Avila who was inside her home. NTSB preliminary report (released July 15, 2026) confirmed electronic data showing the driver manually overrode FSD by pressing the accelerator pedal to 100%, with vehicle speed exceeding 70 mph in a 30 mph residential zone.

The driver, Michael David Butler, 44, was charged with manslaughter. NHTSA has opened a special crash investigation. The NTSB investigation is ongoing.
